Pharmacokinetics
Captopril is rapidly absorbed after oral administration with bioavailability of 70%. It is highly protein bound. It is excreted via Kidneys.
Indications
Captopril inhibits Angiotensin Converting Enzyme.ACE inhibitors are predominantly used as antihypertensives.ACE inhibitors reduce the levels of Angiotensin II and increase the level of Bradykinin. Captopril is used in patients after a Myocardial Infarction. Captopril is also used to treat renal impairment in some diabetic patients as it reduces the proteinuria in diabetics. In addition, it is used to treat congestive heart failure.
Routes of Administration and Dosage
For oral dosage form (tablets): For congestive heart failure: Adults: 12.5 to 25 mg two or three times a day. For hypertension: Adults : 25 to 50 mg two or three times a day. For Renal impairment related to diabetes: Adults : 25 mg three times a day. For treatment after a MI : Adults: 12.5 to 50 mg three times a day.
Contra Indications
Contraindicated in hypersensitivity, Pregnancy, aortic stenosis, significant renal impairment.
Precautions
Allergies: Allergic reaction to captopril may occur. Pregnancy: Studies in animals at doses up to 400 times the human dose have shown that captopril causes an increase in deaths of the foetus and new born. Also, captopril has caused deformed skulls in the offspring of rabbits given doses 2 to 70 times the recommended human dose. Breast-feeding : captopril passes into breast milk.Children : Children may be especially sensitive to the hypotensive effect of ACE inhibitors. This may increase the chance of adverse effects during treatment. Adults: This drug has been tested in patients 65 years of age or older and has not been shown to cause any different side effects or problems in older people than it does in younger adults.
Interactions
Risk of bone marrow depression when used with immunosupressive agents. Causes hyperkalaemia with potassium sparing diuretics. NSAID's may block the hypotensive action of Captopril.
