Pharmacokinetics
Vancomycin is not absorbed orally and intramuscular injection is painful. It acts by inhibition of cell wall synthesis. It is mainly excreted via urine.
Indications
Vancomycin acts against Gram positive cocci and is bactericidal. It acts on Methicillin resistant Staph.Aureus,Staph.epidermis, Strep.pyogenes, Strep.pneumoniae, Enteroccocus faecalis etc.
Routes of Administration and Dosage
Oral dose (capsules, oral solution): For treatment of colitis or severe diarrhoea in Adults:125 to 500 milligrams (mg) every six hours for five to ten days. Children : 10 mg per kilogram (kg) of body weight, up to 125 mg. This dose is taken every six hours for five to ten days.Systemic 0.5 to 1 gram I.V b.d
Contra Indications
Vancomycin is contraindicated in renal and auditory dysfunction.
Precautions
Patients with Renal disease, inflammatory bowel disorders may have an increased chance of adverse effects. Rapid infusion may lead to sudden, profound fall in blood pressure, flushing,and itching (red man syndrome)
Interactions
Cholestyramine, Colestipol : Use of these drugs with oral vancomycin may decrease the effects of oral vancomycin; oral vancomycin and these drugs should be taken several hours apart. Risk of increased nephrotoxicity with concommitant aminoglycosides. Neuromuscular blockade may occur if used together with non-depolarising agents.
