Pharmacokinetics
Roxythromycin is absorbed from small intestine, the drug gets partially destroyed by the gastric juice so it must be administered in enteric coated form. They have better cellular penetration than Erythromycin and can enhance host defence mechanisms. It get mainly concentrated in the bile.
Indications
It is used to treat many kinds of infections including Staph.aureus, Staph.epidermis,Strep.pneumoniae, Cornybacterium diptheriae, Listeria monocytogenes, N.gonorrhea, H.influenza, Chlamydiae, Mycoplasma etc. They are also used to prevent Streptococcal infections in patients with a history of rheumatic heart disease who may be allergic to penicillin.These drugs may also be used to treat Legionnaires' disease.
Routes of Administration and Dosage
Oral: Adults = 150-300mgs bid 30mins before meals for 7 days. Children = 2.5-5mgs/kg every 12hrs for 7 days.
Contra Indications
Contraindication are hypersenitivity, jaundice and pregnancy.
Precautions
In Cardiac disease : High doses of the drug may increase the chance of adverse effects in patients with a history of an arrhythmias. Hepatic disease : Erythromycins, especially erythromycin estolate, may increase the chance of adverse effects involving the liver. Ototoxicity : High doses of roxythromycin may, on rare occasion, cause hearing loss, especially during renal or hepatic disease.
Interactions
With Acetaminophen, Amiodarone, Anabolic steroids, Androgens, Antithyroid agents, Carmustine, Chloroquine, Dantrolene, Daunorubicin, Disulfiram, Divalproex, Estrogens, Etretinate, Gold salts, Hydroxychloroquine, Mercaptopurine, Methotrexate, Methyldopa, Naltrexone : Use of these drugs with Roxythromycin may increase the chance of hepatic impairment.
