Pharmacokinetics
Pentazocine can be given orally, IM or IV. When orally given it has an extensive first pass metabolism . Oral to parental ratio is about 1:4. Pentazocine is conjugated in the liver and excreted in the urine. Plasma t 1/2 is 3-4 hrs,duration of action after a single dose is 4-6 hrs.
Indications
Pentazocine is indicated in moderate to severe pain as in surgery, trauma,fractures, cancer pain, and severe neuralgia. It is used instead of morphine where repeated use of morphine can induce dependency or addiction.
Routes of Administration and Dosage
Oral dose (tablets): As analgesic Adults: 50 mg every three to four hours as needed. Some patients may need 100 mg every three to four hours. The usual maximum dose is 600 mg a day.For injection dosage form: As analgesic in Adults: 30 mg, injected IM, IV or subcutaneously every three to four hours as needed.
Contra Indications
Contraindicated in conditions like hypersensitivity, children under 12 yrs, raised intracranial pressure, hypertension, myocardial infarction, head injury, porphyria, respiratory depression and in narcotic dependency.
Precautions
Some of the adverse effects of pentazocine can be potentially dangerous in patients with CNS disease, head injury, emphysema, asthma, other chronic lung diseases, enlarged prostate and difficulty in micturition. The chance of adverse effects may be increased in patients with Colitis, Heart disease, renal disease and Hepatic disease.
Interactions
Narcotics will not be effective in people taking naltrexone. Pentazocine can increase the sedative effect of alcohol and increase the risk of respiratory depression when used with halothane. It is incompatible with soluble barbiturate.
