Pharmacokinetics
Nitrofurantoin is completely absorbed from the GIT and food favours its absorption. It doesn't reach therapeutic concentration in plasma but only in urine hence used in UTI. Its activity get lowered in alkaline urine.
Indications
Nitrofurantoin belongs to the family of drugs called anti-infectives. It is used to treat infections of the urinary tract.
Routes of Administration and Dosage
Oral dose: Adults and adolescents: 50 to 100 mg every six hours. Children up to 1 month of age: Use is not recommended. For the extended-release capsule dosage form: Adults and children 12 years of age and older: 100 mg every twelve hours for seven days.
Contra Indications
Contraindicated in infants under 1 year of age, severe oliguria, anuria and in renal failure.
Precautions
In Glucose-6-phosphate dehydrogenase (G6PD) deficiency: Nitrofurantoin may cause anemia in patients with G6PD deficiency. In renal disease (other than infection): The chance of side effects of this drug may be increased and the drug may be less effective in patients with renal disease. Pulmonary disease and nerve damage : Patients with pulmonary disease or nerve damage may have an increase in adverse effects when they take nitrofurantoin.
Interactions
With Antidiabetics, Dapsone, Methyldopa, Primaquine, Procainamide, Quinidine,Sulfonamides,Sulfoxone,Vitamin K : Patients who take nitrofurantoin with any of these drugs develop blood disorders . With Carbamazepine, Chloroquine, Cisplatin, Diphtheria, tetanus, and pertussis (DTP) vaccine, Disulfiram, Ethotoin, Hydroxychloroquine, Lindane, Lithium and Vincristine: Patients who take nitrofurantoin with any of these drugs, or who have received a DTP vaccine within the last 30 days or are going to receive a DTP may have an increase in neurological adverse effects.
