Pharmacokinetics
The bio-availablity of hydrocortisone is about 50 % after oral administration. It is metabolised in the hepatic microsomal enzymes. It is 90% plasma protein bound. The plasma half life is 1.5 hours.
Indications
Hydrocortisone is used to reduce inflammation . It reduces swelling, erythema, pruritis, and allergic reactions. It is used in the treatment of severe allergies,skin problems and arthritis.It is used in shock, status asthmaticus and acute adrenal insufficiency.
Routes of Administration and Dosage
Oral:20 mg in the morning+10 mgs in the afternoon. Parenteral:100 mgs I.V bolus followed by 100 mgs 8 hourly infusion.
Contra Indications
Hydrocortisone is cotraindicated in viral/fungal infections, tubercular or syphilitic lesions, bacterial infections unless used in conjuction with appropriate chemotherapy.
Precautions
Precaution has to be taken in CCF, hypertension, diabetes, epilepsy, CRF and in elderly. Longterm use of Hydrocotisone can cause cushing's habitus, hyperglycemia, muscular weakness, increased susceptibility to infection, delayed wound healing, and psychiatric disturbances.
Interactions
Hydrocortisone has reduced efficacy with concurrent use of carbamazepine, phenytoin, primidone and barbiturates. It has enhanced effect in women taking oral contraceptives.
