Pharmacokinetics
The loop agents are rapidly absorbed and handled by renal excretion and secretion.Duration of maximal action is for 3 to 4 hrs.
Indications
Ethacrynic Acid is also used to treat hypertension in those patients who are not helped by other drugs or in those patients who have renal insufficiency.
Routes of Administration and Dosage
For ethacrynic acid: Oral dose (oral solution or tablets): Diuretic: Adults: 50 to 200 milligrams (mg) a day. This may be taken as a single dose or divided into smaller doses. Children: 25 mg a day. For injection dosage form: Diuretic: Adults: 50 mg injected into a vein every two to six hours as needed. Children: The usual dose is 1 mg per kilogram (kg) of body weight injected IV.
Contra Indications
It is contraindicated in anuria, those with history of allergy to ethacrynic acid, hypokalaemia, hyponatraemia, precoma stage associated with hepatic cirrhosis and Addisons disease.
Precautions
Allergies : Allergic reaction to ethacrynic acid may occur. Pregnancy:In general, diuretics are not useful for edema of feet and hands that occurs during pregnancy. Diuretics should not be taken during pregnancy. Breast-feeding: Ethacrynic acid passes into breast milk. Older adults: Dizziness, lightheadedness, or signs of hypokalemia may be more likely to occur in the elderly, who are more sensitive to the effects of loop diuretics. Elderly patients may also be more likely to develop thrombii.
Interactions
Loop Diuretics increase the nephrotoxicity of Cephalosporins and aminoglycosides. Phenytoin and Indomethacin decrease the effect of loop diuretics. Analgesics may reduce the natriuretic effect. Loop Diuretics enhance the activity of muscle relaxants.
