Pharmacokinetics D-penicillamine is a degradation product of penicillin. Only the d-isomer is used because l-isomer causes optic neuritis. It is slowly but adequately absorbed after oral administration. The drug gets metabolised in the body and is excreted in the urine and faeces. Indications D-penicillamine is used in severe and active rheumatoid arthritis, it is used mainly as chelating agent in heavy metal poisoning as in lead, mercury and copper poisoning. It is used in Wilson's disease. Routes of Administration and Dosage Oral dose : For Rheumatoid arthritis in adults : Initially 125-250mg daily before food for one month, increased by similar amounts at 4 weeks interval to a maintenance dose of 500-750mg. In elderly: Initially 125mg daily before food for one month, incre Contra Indications The drug is contraindicated in hypersensitivity, SLE, Thrombocytopenia, agranulocytosis and during lactation. Precautions When taking D-penicillamine, blood count, renal and hepatic function should be periodically monitored. During pregnancy the drug is generally avoided except in Wilson's disease and cystinuria. Interactions When using D-penicillamine drugs like gold, Chloroquine, and immunosupressive agents should be avoided. Iron, Antacids and zinc interferes with absorption of D-penicillamine.
